Path and filename of at least one catalog file.
This catalog file should reference all other files
named 'catalog' or 'docbook.cat'.
At least for DIY setups it's easier this way.
For example this is a working master catalog on w2k:
CATALOG "C:/usr/jade-1.2.1/catalog"
CATALOG "F:/docbook/docbook-dsssl-1.77/catalog"
CATALOG "F:/docbook/DocBook-4.1/docbook.cat"
CATALOG "F:/docbook/docbook-4.2CR3/docbook.cat"
CATALOG "F:/docbook/DocBookXML-4.1.2/docbook.cat"
The referenced files are unmodified.

Hard to say. On RedHat and Debian this stuff works
out of the box. No need to modify anything.
