Your doctype declaration looks very strange.
AFAIK the correct string is:
<!DOCTYPE book PUBLIC "-//OASIS//DTD DocBook V4.1//EN"
At least on my RedHat 7.2 that matches a PUBLIC
statement of
/usr/share/sgml/docbook/sgml-dtd-4.1/catalog
