I believe that is considered a style element,
so it would need to be done in XSL or DSSSL.

Hello Everybody,

Can I defined in Docbook that each page of a document has a header and
footer?  I'm thinking of some article or articleinfo child-tag or
something...  I'm checking the reference (list of all tags) but I can't
find anything.  Probably focussing on the wrong keyword...
