Look at the generated TeX-file and see whether you find a line like:

\usepackage{fancyvrb}

If not, make sure you have:

<xsl:param name="latex.use.fancyvrb">1</xsl:param>

in your custom XSL file (this is default anyway, but maybe you switched it 
off) and regenerate the TeX-file.
