I suspect the problem is with the stylesheet's decision to allow
objects to float.  If you want objects (e.g. tables, figures,
examples) to be inserted exactly where defined in the text, you
need to customize/modify the stylesheet.

Specifically, this portion of ldp.dsl is responsible:

(define formal-object-float
  ;; Do formal objects float?
  #t)
