As stated in previous posts of mine, I adopted the <bridgehead> tag for use in
burying the revision history in an appendix. Unfortunately, that particular
tag has some problems.
My current method of choice is simply to use a frameless table, like so.

I've dropped the field for the initials of the person submitting the revision, as it's redundant when it's the author.
