This is caused by using sgml2html rather than db2html (or docbook2html).
Give it a shot with docbook2* instead.

Yep, same thing, sgml2html doesn't know about this kind of document.
Give it a shot with the db2* tools, and let's see what that fixes.  If
things aren't hunky-dory afterwards, let us know, and we'll see what we
turn up.
