I like the full list of links provided by Stein's version. I think it will
be difficult to do the list of keywords though. How would this information
be extracted from the documents? Maybe once there is a standardized CMS
that stores the meta-data this is something that authors could be asked to
provide for their documents.
The feedback links are interesting, but i don't think Tille will want the
feedback to go to feedback@. I know in the Author Guide all the feedback@
addresses were changed to discuss@. I'd also combine the author links into
a single, "Feedback to the author" and then in brackets "(positive feedback
and corrections are welcome)" or something like that.

I'd say it's more of a table of contents than an index...you say potayto I
