Are you converting to XHTML or HTML? I think there is a problem with
the DocBook-XSL stylesheets that produces the characters you describe
when converting to XHTML. Unfortunately I cannot remember the
workaround. Try using the stylesheets for HTML instead and see if the
problem goes away.
