The Table of Content is added by the XSL stylesheet
The Notes sections is being produced by the following
XML in your document:

<note role="date"><para>October 26, 2004</para></note>
</para>
I would probably change it to something like below
(offcourse use your own name/title instead of Emma's
:) )
