I was able to find an old copy of the SGML source for the revision
denoted above (authored by Wayne Leister). I have restored
that version of the document, marked at as being 'archived'
(for being "severely out of date"), and added it to the list of
unmaintained documents.
Hopefully, someone will be interested in maintaining this HOWTO.
