You can use <sect1>, <sect2>, <sect3>, etc. for sections and
subsections. <sect1> is the top level and they are nested like this:
<sect1>
<sect2>
<sect3>
<sect4>
</sect4>
</sect3>
</sect2>
</sect1>

I'm guessing this is probably happening with tags like <programlisting>
or <screen>. These are formatted similar to the HTML <pre> tag in that
whitespace *does* matter. The only way I know to fix it for these types
of tags is to either shorten the lines in your document or hack the
stylesheet to use a smaller font size.
