One problem: You are specifying a DocBook DTD and using linuxdoc to
build the SGML. AFAIK, the linuxdoc command only works with a linuxdoc
DTD (different). I would recommend learning DocBook though tldp will
accept either types of documents. 

Command I use to build multi-page HTML:

 jade -t sgml -ihtml -d /usr/lib/sgml/stylesheets/ldp.dsl\#html

That uses the TLDP stylesheet (ldp.dsl), which is available on the
tldp website somewhere, and would need to be installed manually. There
also is decent instructions in the Author's guide there somewhere. 

The ldp stylesheet is not necessary, but its the same one used by
TLDP, so your test builds would look the same.

You could try the 'db2html' script also.

Your mark-up looked good AFAICS. Only one minor problem (an unclosed
para tag).
