Sorry.  I just looked at an old reference manual for linuxdoc and it
pointed out that the index only works for latex format (using
sgml2latex).  Furthermore, it's use requires some manual steps but the
author said he was working to fix that.  Did he?  But even if he did,
it's still only good for latex format.

So it wasn't a good example.  I'm guessing that in the index one finds 
"disk" as the main subject and "introduction" as a secondary subject
under disk.
