If you are using xsltproc, you can generate an index as follows:

  <index id="bookindex"></index>

As long as you have <indexterm/>s in your document, you should see a
full-fledged index at the point in the document where you drop this
<index/> tag.

( Hmm....I could probably use <index id="bookindex"/>.  I wonder why
  I didn't. )
