OK, so what is proper use of the indexing terms? What is obvious
is the structure "something!somethingelse" but just what is proper
use of the words before and after the exclamation mark(s)?

Moreover, how can one generate an index in LinuxDoc (and DocBook)?

I looked in the author guides but didn't find anything.
