The DSL-HOWTO was while reading with Acrobat from tldp.org docs. It
may be something different, since all worked well until I bounced out
of the TOC several times. So it may be something acrobat was doing.

I have some good pointers in other replies. Based on the fact that
this does not seem commonplace, I am guessing it is markup or
stylesheet related, and I can workaround it.
