Have a look at htmldoc from www.easysw.com. It is under a GPL license, though
they charge for support. It lets you edit HTML with any tool you choose, check
it with a browser, then convert a bunch of HTML files to any of:

	HTML with previous/table of contents/next links + table of contents file
        (e.g. http://www.freeswan.org/freeswan_trees/freeswan-1.8/doc/index.html)
	one big HTML file
	PDF, including a document map
	Postscript

More-or-less the same outputs you can get from DocBook. Less flexible, but easier.

There's a Windows version of htmldoc, though it is somewhat hard to find on
their website. Works fine for me on both 98 and Linux.
