What you are trying to do, if I understand it correctly, will not
work. Is the document DocBook sgml?
Daniel Veillard added the --docbook flag to xsltproc as, in his words,
"an ugly, bad bad hack" to try to help people who wanted to process
DocBook sgml with his tools. But using his xml tools to process sgml is
rather like using the handle of your screwdriver to pound in a nail -
it will work if conditions are perfect, but they rarely are. One thing
it doesn't handle is entities, according to Daniel.
Your best solution if you want to use xsltproc is to convert your doc
to xml.
