as long as I know, there are three sets of tools that gives different results:
sgmltools -b xxx. These seems to recognise docbook fine (on my suse7.2) but
don't write toc
docbook2xxx. refuses my LyX generated docbook code, gives the best toc
db2xxx gives a very good result. My choice. small toc (nice for my very small
document)
