Docbook takes care of 1 and 2 quite easily and the LDP Author Guide
covers the remainder. I know this as I generated PDFs which I then
printed and bound as books and took to a number of conferences. As Karl
has already indicated, a majority of the early Linux books were merely
reprints of LDP material.
